The birth of every child born in the Province of Ontario must be registered within 30 days after the date of birth with the Municipal Clerk in the municipality in which the child was born. A delay in registering your child’s birth could affect his/her Ontario Health Insurance Benefits.

In order to register your child’s birth, you are required to complete a Statement of Live Birth and return it to the Deputy Division Registrar at the Municipality of Central Huron. The Statement of Live Birth is distributed to new mothers by the hospital or midwife services.

In order to avoid any delays in the process, please follow these instructions and those attached to the Statement of Live Birth.

  • Complete all sections of the form. Please be sure to answer item 20a.
  • Should any information not be available (i.e. the identity of the father) indicate it by stating information not provided.
  • Should you have any corrections, cross it out with a single line, insert the correct information and initials by both parents (except where the father is not acknowledged). Please do not use brackets.
  • The use of underlining, brackets, arrows, red ink, correction fluid, or any other method of correction other than indicated above is not accepted by the Ministry and a new form must be completed.

In a separate process, medical information is reported to the Municipality to support the Statement of Live Birth. This documentation is then forwarded to the Office of the Registrar General. This process can take anywhere from four weeks to three months.

Adults taking children across international borders are often required to prove that they have the right to do so. Parents travelling with a new born before a birth certificate has been issued may obtain an appropriate letter from their local municipal office. The cost of this service is $10.00. (Can only be issued if registration process has been completed at local municipality and must be within 90 days of date of registration . Identification is required).

Applications to obtain an Ontario Birth Certificate or Death Certificate, can be obtained from the Municipal office. Applicable fees charged by the Province are set out on the application form. The completed application forms, together with the appropriate fee, must be mailed to the Office of the Registrar General, 189 Red River Road, P.O. Box 4600, Thunder Bay, Ontario, P7B 6L8.
